IV THERAPY & WELLNESS MEMBERSHIP TERMS & CONDITIONS
You agree to be charged on a recurring basis for the term set out by Crystal’s Essentials, LLC. In the event that your payment fails, you and Crystal’s Essentials, LLC will be notified. We will attempt to take payment for the membership the day before it expires, and if it fails, again after 24 hours, 48 hours and finally after 4 days. If all these payments fail, your membership will automatically be cancelled.
A valid ID and credit card are required to enroll in Crystal’s Essentials, LLC monthly membership. Cash payments will not be accepted. Membership contracts are a minimum of 3 months, and by completing purchase of this membership, you are giving Crystal’s Essentials, LLC consent to auto-renew your membership every month by charging the card that you have placed on file with this purchase for a minimum of months and until you cancel. Membership dues will be charged to your credit card on file on the date you signed up each month (i.e., If you sign up on April 15th, you will be charged on 15 April, 15 May, 15 June, etc. until cancelled. Membership benefits are non-refundable and non-transferable and missed injections and/or infusions do NOT transfer or rollover to the next month. Monthly memberships may not be used in combination with any other packages, promotional offers, or discount coupons.
APPOINTMENTS: Membership therapies are valid only during normal business hours. You are responsible for scheduling your weekly or biweekly service(s) in advance to ensure that you receive your preferred date and time. Appointments at Crystal’s Essentials, LLC are made on a first-come, first-serve basis. However, you will have priority, same day booking if possible. Therefore, if you are not able to schedule in advance just call, and we will schedule you as soon as possible that day. If you are unable to complete all your appointments within the month timeframe given, you will forfeit any remaining injections and/or infusions for that 30-day period. Per our appointment cancellation policy, please provide 24-hour notice of appointment cancellation to avoid any appointment cancellation fees.
MEMBERSHIP CANCELLATIONS: A 30-day notice is required for cancellations. Cancellation of membership under 3 months will result in a $150 cancellation fee. To cancel your membership, a written notice is required and can be sent to hello@crystalsessentials.com. Crystal’s Essentials, LLC reserves the right to cancel any membership at any time. Crystal’s Essentials, LLC reserves the right to adjust membership pricing or membership services with a 30-day written notice.
Crystal’s Essentials, LLC does NOT have a one-month membership. Membership is valid only for the pre-paid 30-day calendar period. By completing the purchase of this membership, you certify that you have read the foregoing Membership Agreement and acknowledge that you fully understand and agree to be bound by all the terms and conditions hereof regarding Crystal’s Essentials, LLC monthly memberships. You further acknowledge that a fully executed copy of this Membership Agreement has been provided to you.
